The official report is out&#8230;me, I would describe it ugly.<\/p>\n<blockquote><p>The scathing crash investigation report shared with Military.com pointed to &#8220;failure to perform standard crew resource management,&#8221; along with adverse weather conditions, ineffective flying operations supervision, lack of awareness, and &#8220;an unhealthy organizational culture that permitted degradation of airmanship skills&#8221; as contributing factors in the Jan. 4 crash.<\/p>\n<p>The B-1B Lancer, which was on a training mission, crashed roughly 100 feet shy of the runway, skidded more than 5,000 feet down the tarmac, and was then engulfed in flames from the crash. The four crew members all ejected, but two of them suffered injuries as a result. Both were medically treated and later released, according to Air Force Global Strike Command, which commissioned the crash investigation report. The Lancer was destroyed, and the damage to the aircraft and the runway was estimated to be more than $456 million.<\/p>\n<p>The report also points out that one crew member who was injured during the ejection was not wearing all the proper flight equipment. And the other injured crew member&#8217;s weight was reportedly above both the ejection seat&#8217;s recommended limit of 211 pounds and the Air Force&#8217;s recommendation of 245 pounds.<\/p>\n<p>&#8220;The preponderance of the evidence revealed an ineffective and unhealthy culture, which directly contributed to the mishap,&#8221; he wrote. &#8220;Specifically, the [34th Bomb Squadron&#8217;s] overall lack of discipline, inadequate focus on basic airmanship skills, and failure to properly identify and mitigate risk, coupled with the [28th Operations Support Squadron&#8217;s] ineffective communication, inadequate program management, and lack of supervisory oversight, set conditions that allowed this mishap to occur by directly leading to the mishap&#8217;s cause and its three non-weather-related, substantially contributing factors.&#8221;\u00a0 <a href=\"https:\/\/www.military.com\/daily-news\/2024\/07\/25\/report-unhealthy-culture-ellsworth-air-force-base-units-contributed-b-1b-lancer-crash.html?ESRC=eb_240726.nl&amp;utm_medium=email&amp;utm_source=eb&amp;utm_campaign=20240726\">Military.com<\/a><\/p><\/blockquote>\n<p>In defense of the injured crewmember, they may not MAKE the equipment large enough for a 260 pounder &#8216;they&#8217;.<\/p>\n<p>Seems the only accusation the board missed was &#8220;you&#8217;re ugly and your mother dresses you funny.&#8221;\u00a0 I foresee a LOT of truncated careers&#8230;<\/p>\n<p><img loading=\"lazy\" decoding=\"async\" class=\"size-medium wp-image-147685 aligncenter\" src=\"https:\/\/www.azuse.cloud\/wp-content\/uploads\/2023\/09\/sale-287x300.png\" alt=\"\" width=\"287\" height=\"300\" srcset=\"https:\/\/www.azuse.cloud\/wp-content\/uploads\/2023\/09\/sale-287x300.png 287w, https:\/\/www.azuse.cloud\/wp-content\/uploads\/2023\/09\/sale-318x333.png 318w, https:\/\/www.azuse.cloud\/wp-content\/uploads\/2023\/09\/sale.png 516w\" sizes=\"auto, (max-width: 287px) 100vw, 287px\" \/><\/p>\n<p>Looks like after the CDC loosened their rules, some of the airlines are loosening their rules on dogs being able to accompany their owners on planes.<\/p>\n<blockquote>\n<p class=\"Paragraph-sc-1tqpf5s-0 kEzXdV body-paragraph body-paragraph\">The airline used by many overseas military families to transport their dogs back to the United States will now allow at least some dogs to fly on the same flight as their owner, a move that follows the loosening of some government restrictions last week.<\/p>\n<p class=\"Paragraph-sc-1tqpf5s-0 kEzXdV body-paragraph body-paragraph\">Eight airlines, including German airline Lufthansa, had announced they would no longer allow dogs to fly in the cabin or cargo hold on the same flight as their owner <a href=\"https:\/\/www.militarytimes.com\/news\/your-military\/2024\/07\/22\/military-families-overseas-scrambling-as-us-dog-travel-rules-change\/\" target=\"_blank\" rel=\"noopener\">due to new dog importing rules<\/a> issued by the Centers for Disease Control and Prevention.<\/p>\n<p class=\"Paragraph-sc-1tqpf5s-0 kEzXdV body-paragraph body-paragraph\">In an email response to Military Times on July 26, Lufthansa spokesman Joerg Waber noted that \u201cexisting bookings for the transportation of dogs remain unaffected, provided that all CDC requirements are met.\u201d<\/p>\n<p class=\"Paragraph-sc-1tqpf5s-0 kEzXdV body-paragraph body-paragraph\">That includes bookings previously made for flights scheduled for August or later. Many military families overseas book flights months in advance.<\/p>\n<p class=\"Paragraph-sc-1tqpf5s-0 kEzXdV body-paragraph body-paragraph\">New Lufthansa bookings for dogs on or after Aug. 1 \u201cwill only be accepted for the airports in Washington, D.C., New York-JFK, Atlanta, Miami, Los Angeles and Philadelphia that provide the necessary infrastructure and meet criteria required by the CDC in case the animal is denied direct entry to the U.S.,\u201d Waber added.\u00a0 <a href=\"https:\/\/www.militarytimes.com\/news\/your-military\/2024\/07\/29\/more-dogs-allowed-lufthansa-reopens-options-for-some-flying-to-us\/?utm_source=sailthru&amp;utm_medium=email&amp;utm_campaign=army-dnr\">Military Times<\/a><\/p>\n<\/blockquote>\n<p>Not a complete victory, but it should at least help some of the families.
